In Delray Beach, Florida they get it. They care. They understand persons suffering from Alzheimer's are likely to wander.

Statistics show that 60 percent, over 3 million, Alzheimer's sufferers are likely to wander.

It won't cost you 50 bucks to sign up and get into the system. You won't have to pay 25 bucks a year to keep the service active.

All you will need to do, if you live in Delray Beach, is go down to the Delray Beach police station, get registered and get a picture taken.
"We're building a databank of photographs and information," said John Evans, a sergeant with the Delray Beach Police.
Is this a perfect solution? No. It certainly is better than no solution.

The price is right and it shows the problem is being recognized here in Palm Beach county.

I'm feeling agitated. This is when my stomach starts bothering me. I am also suffering from a case of deja vu.

Yesterday I published an article -- Study of aging in Group Health patients aims to prevent dementia, including Alzheimer's -- and the following words are stuck in my mind.
When older people cannot walk except very slowly, this predicts that they are likely to become "frail" (weak and prone to diseases) and develop dementia.
This reminds me of when I first noticed my mother was scrapping her feet on the ground. A sound that was so disconcerting it had me worried. My sister Joanne also noticed the sound.

I asked everyone, what do you think? They all said the same thing -- she's getting old.

I accepted this. Nevertheless, for more than a year I was thinking about it all the time.

You might ask -- well why didn't you take her to the doctor? She went to the doctor five times during that year. Guess what her doctor said when I finally asked him about it. Good guess -- she's getting old.

I wrote about walking and balance many times on this website -- Changes in Walking and Balance Can be a Sign of Alzheimer's and Dementia.
...early detection of balance problems may lead to earlier detection of Alzheimer's disease or other forms of dementia.

Thinking about this led me to envision an advertisement for Alzheimer's Awareness.

Something that might actually catch the attention of people that are worried about their elderly loved one. People that suspect something is wrong, or going wrong. An advertisement that might have some impact, or make a difference.

Here is my television script.

Ad comes on. Black screen. The sound of my mother's feet scrapping across the concrete. That eerie sound.

The picture comes on. A shot of my mother and me walking -- I am holding her hand. We are walking very slowly.

I turn and start talking.

My name is Bob DeMarco, I am an Alzheimer's caregiver.

I put may arm around my mother's shoulder. This is my mother Dorothy, now 93 years old, she suffers from Alzheimer's disease.

Prior to her dementia diagnosis my mother started scrapping her feet on the ground. In 2002, she walked 30 blocks in New York city without a problem. In late 2003, I finally realized she couldn't walk a block. She fell and couldn't get up without assistance.

At the same time, she was still driving her car. Still doing her own shopping. Still taking herself to bingo on Wednesday night. And, still living on her own.

Finally, I had her doctor give her a physical. She was in good health. He explained the problem to me this way -- she is getting old. He told me he could order her a walker.

A year and three doctors later we learned that my mother was suffering from dementia -- probable Alzheimer's disease.

Don't make the same mistake I made. If your elderly loved one starts scraping their feet on the ground, starts walking slowly, or starts having problems with their balance -- get their memory tested. Get their memory tested by a specialist -- not your family doctor.

Early detection of Alzheimer's or dementia is imperative to maintain a good quality of life. Early detection and treatment makes a difference.

Just ask yourself this question.

If you were in an early stage of Alzheimer's would you want your family to have you tested?

Or, would you rather fall into the black hole of dementia while everyone sits around saying -- she is just getting old.

The television ad closes with an 800 number that actually goes to someone that can tell you where and how to get tested, and the sound of my mother's feet scrapping the concrete.

Who knows, maybe sell some Aricept while we are at it.

Ask the next ten people you see that are not involved in medicine or caregiving if they know which month is Alzheimer's awareness month, and when they hold national Alzheimer's screening day.

The joint project between Group Health Research Institute and the University of Washington (UW) focuses on finding ways to delay or prevent dementia, including Alzheimer's disease, and declines in memory and thinking. It aims to deepen understanding of how the body—especially the brain—ages.....
The ACT study has learned many lessons to date, published in nearly 250 scientific articles. The study is known best for findings about the benefits of physical activity:
  • Regular exercise is linked to reduced risk of dementia, Alzheimer's disease, and declines in how well people think.
  • The first signs of dementia—including Alzheimer's disease—may be physical, not mental.
  • For example, when older people cannot walk except very slowly, this predicts that they are likely to become "frail" (weak and prone to diseases) and develop dementia.

At each biannual study visit, participants are observed as they walk and do many other physical and mental tasks. They are asked many questions, including whether they would allow an autopsy on their brains after they die. Thanks to willing participants, the study has gleaned its most recent discoveries, which center on how vascular (blood vessel) disease contributes to late-life dementia, including Alzheimer's disease:
  • Microscopic injuries to blood vessels in the brain, called cerebral "microinfarcts," which cause "small-vessel disease of the brain," can lead to many brain problems in late life.
  • These microinfarcts may be responsible for more cases of dementia than previously recognized, often combined with Alzheimer's.
  • Controlling high blood pressure may lower the risk of dementia by minimizing microinfarcts.
In the next five years, the ACT team will largely focus on deeper exploration of these most recent findings. They will test whether diabetes, kidney disease, and atrial fibrillation (the most common abnormal heart rhythm) contribute to brain decline through microinfarcts or changes in larger blood vessels—and whether physical activity counters them.

Ongoing trials also include interventions to improve sleep as well as thinking and physical well-being.

Like the celebrated Framingham Heart Study, ACT is a longitudinal cohort study of people leading their lives in a community. That means it follows a large group of people at regular intervals over a long time to see what happens to them.

As with Framingham, insights from the study group have helped other people to stay healthier. ACT the only 'population-based' study involving autopsy and study of diseases of nervous system tissue (neuropathology).

The ACT study has learned many lessons to date, published in nearly 250 scientific articles. The study is known best for findings about the benefits of physical activity:
  • Regular exercise is linked to reduced risk of dementia, Alzheimer's disease, and declines in how well people think.
  • The first signs of dementia—including Alzheimer's disease—may be physical, not mental.
  • For example, when older people cannot walk except very slowly, this predicts that they are likely to become "frail" (weak and prone to diseases) and develop dementia.
Other study findings have led to changes in how people with dementia are cared for and helped ease the burdens these illnesses can cause. Here are some examples:
  • Certain drugs, especially sedatives, can cause confusion and falling.
  • Statin drugs, used to treat high cholesterol, may be linked to fewer of the microscopic brain changes that are typical of Alzheimer's disease.
  • Anti-oxidant supplements (vitamins A and C) and some common pain relievers (non-steroidal anti-inflammatory drugs, or NSAIDs) seem not to prevent brain decline.
  • But a related study of Japanese-American seniors in King County linked regularly drinking fruit and vegetable juices—both rich sources of natural anti-oxidants—to reduced risk of Alzheimer's disease.
  • Avoiding environmental toxins throughout life can help delay the brain's decline.
  • Wearing shoes—not just socks, stockings, or slippery slippers—can help prevent falling, even at home.
  • Wandering, agitation, and disturbed sleep complicate the lives of patients with Alzheimer's and their families.
The ACT study enrolls about 240 new people each year, keeping the study group size steady at about 2,000 people. People are invited to participate after being randomly selected from a census of all Group Health members who are age 65, enrolled in central King County medical centers, and without dementia.

The participants have been patients at Group Health for an average of at least 30 years before joining the study. "That gives us a wealth of medical, drug, and lifestyle information to help answer our research questions," Dr. Larson said. "ACT is the world's only study that can link outcomes for dementia, frailty, and aging to data that captures each participant's whole health history, including medical, lab, and pharmacy records."

MDVN Starts New Alzheimer’s Trials

Earlier this month, Medivation, Inc. (MDVN - Analyst Report) and Pfizer, Inc. (PFE - Analyst Report) initiated two phase III studies, CONTACT and CONSTELLATION, with Dimebon in patients with moderate-to-severe Alzheimer's disease (AD). Dimebon is being developed in collaboration with Pfizer for the treatment of Alzheimer’s and Huntington diseases.

The CONTACT study will assess the potential benefits of adding Dimebon to ongoing treatment with Pfizer’s Aricept on neuropsychiatric symptoms and activities of daily living. Meanwhile, the CONSTELLATION study will evaluate the effects of adding Dimebon to Forest Lab’s (FRX - Analyst Report) Namenda, another standard of care, on cognition, memory and activities of daily living.

Preclinical studies indicate that Dimebon has the potential to protect brain cells from damage and enhance brain cell survival, by stabilizing and improving mitochondrial function. Dimebon’s mechanism is distinct from currently available Alzheimer’s medications.

Dimebon is currently in seven studies which are evaluating the safety and efficacy of the candidate across all stages of Alzheimer's disease, both as monotherapy as well as in combination with currently available Alzheimer's treatments, and in Huntington disease.

Dimebon’s unique mechanism of action and efficacy seen in clinical trials could make it a significant player in the worldwide Alzheimer’s market, which represents huge commercial potential. It is estimated that the market is currently worth about $5 billion.

Dimebon has successfully completed the first of two pivotal trials required to gain marketing approval in the U.S. for mild-to-moderate Alzheimer’s disease. A second confirmatory phase III study, CONNECTION, is currently ongoing. We expect to see top-line results from this study in the first half of 2010. Positive results should allow Medivation to go ahead with the filing of the new drug application (NDA) for the Alzheimer’s indication in 2011.

We currently have a Neutral recommendation on Medivation.

She has been replaced by a woman who says bigoted outrageous things, gets angry if she's contradicted, and often acts like a spoiled child.

My visit did not go well.

I was not prepared for the changes that have occurred in my mom since I last visited in December. I was impatient and angry with her. I could not label it "the illness" when she behaved badly but instead reacted as if she were unimpaired. I tried to reason with her. I tried to shame her into acting better. I got angry with her. And it's not her fault.

Yesterday I had tea with a friend. I related to her my various shortcomings in dealing with my mom. She asked me if perhaps I was sublimating my sadness into anger. The tears welled in my eyes, and I knew she was right.

The Alzheimer's Association has a new GPS-based program to help find dementia patients who wander.  It seems like an upgrade to the Safe Return program, which I purchased for my dad.  (I still have his bracelet--I don't know why I wanted it.)
Rather than try to explain it, here's what they sent me:

The Alzheimer's Association Comfort Zone™, powered by Omnilink, is the first comprehensive location management system designed specifically for Alzheimer’s, giving people with the disease more freedom and independence in their community while providing their family some peace of mind.  Comfort Zone is a Web-based application that works with various location devices throughout the progression of the disease to proactively communicate the location of the person with Alzheimer’s within two to 30 minutes.  Comfort Zone also offers families assistance with 24/7 monitoring center services and access to emergency health records from the MedicAlert Foundation.

The price of the Comfort Zone service packages varies, beginning at $42.99 a month with a $45.00 activation fee, similar to most cell phone services.  For more information about Comfort Zone please see the attached press release and fact sheets.  You can also visit the Alzheimer’s Association at www.alz.org/comfortzone
At first I was like, $42 a month!  The bracelet I bought my dad was a 1-time $45 fee!  (I think that's changed now, to a per year fee, but I could be wrong.)  Then I reread the most important phrase:
communicate the location of the person with Alzheimer’s within two to 30 minutes
No driving around aimlessly.  No having squadrons of police offices searching.   It doesn't matter if it's dark or cold, GPS will find your loved one.  If that's not worth $42 a month, I don't know what is.  I only had to search for my dad twice--and yes, it was dark and cold one of those time--and while I was driving around, my cell phone illegally in hand, I would have paid any amount of money to just find my dad safe.

Just came across this article on CBS, saying that the number of Alzheimer's sufferers is doubling every 20 years. Currently 35 million people world wide are afflicted, which is 10% more than had been estimated.
Barring a medical breakthrough, the World Alzheimer Report projects dementia will nearly double every 20 years. By 2050, it will affect a staggering 115.4 million people, the report concludes. ....
The report urges the World Health Organization to declare dementia a health priority and for national governments to follow suit. It recommends major new investments in research to uncover what causes dementia and how to slow, if not stop, the creeping brain disease that gradually robs sufferers of their memories and ability to care for themselves, eventually killing them.
There is no known cure; today's drugs only temporarily alleviate symptoms. Scientists aren't even sure what causes Alzheimer's. 
I have no affiliation with any of the studies I suggest people enroll in..but only by being human guinea pigs can new drugs be tested that can save other people.

Jeannette Twomey, inventor of the Kind Reminder, sent me an email recently to tell me about her product.  It's a little device that plays a recorded message with the push of a large button.  The caregiver can leave a simple message like, "It's Tuesday and I'll see you at 5 p.m." or any information that the dementia patient may need to access frequently.
The recorder is inexpensive and 5% of the purchase price goes to the Alzheimer's Association. If my dad was still alive, I'd have no problem shelling out $20 for this!  Maybe if he had had a recording of my voice to carry around, he wouldn't have forgotten me.

Me: Mom, when we went to see the doctor he was very concerned about your being all alone in your house. He wanted you to have Diane in during the day and the evening.

Mom: He can't tell me what to do. I'm independent. I've been independent all of my life. I'll get another doctor.

Me: Another doctor will tell you the same thing.

Mom: How will he know?

Me: Your records go with you, and your doctor has been writing everything down. You know, if you don't do what you agreed to do, the doctor may put you in the nursing home.

Mom: He can't do that. I'm not going in the nursing home. I'll call the police and tell them what he's trying to do.

Every time I talk to her she tells me she's doing great and that she hasn't fallen again. Last week she told me that she's doing so well that she doesn't need Diane more than one day a week. If she actually follows through, I'll have to go out to see her and assess the situation. I really don't want to force her into the nursing home, but she only becomes more dangerous to herself as time goes on.

I haven't been to see her since December. That was the deadline I had set for her to make a decision either to move to my town or go into the nursing home. I was pretty sure she wouldn't move to my town, so I had talked to her doctor about how we could get her into the nursing home. The plan was to take her in to see him, and he would tell her she had to go to the hospital for testing. After completing testing at the hospital she would be transferred to the nursing home for "rehabilitation."

But when I started thinking about the fight she might put up and how miserable she would be in the nursing home, I wasn't able to do it. I was also concerned that my uncle might remove her if she asked him.

At this time she also was getting regular help at home. Last August I finally found someone to go in and help her. Diane is a young woman who has some home health care experience and lives in the same town as my mom. At the time I hired her I had her go in Monday through Friday because my mom had a problem with her foot and needed encouragement to walk on it.

Before I traveled to my mom's in December I talked to her doctor about the situation. He believed that with the proper support she could live at home a while longer. He wanted her to have Diane for additional time every day - at least two hours during the day and two hours every evening. He also wanted her to get Lifeline and diabetic shoes so that she would be more stable on her feet.

When I took my mom to see the doctor she agreed to everything. She even agreed to pay Diane for the evening hours. At this time I was paying Diane out of my mom's trust because I thought it was temporary and that my mom would be going into the nursing home. Since we decided to delay her going into the nursing home, I needed to have her start paying Diane so that I could conserve the funds in the trust. The first step was to have her pay the evening hours, and later I would have her pay for all Diane's hours. (Of course my mom didn't know I was paying Diane out of the trust. Diane told her she was working for an agency and that there wasn't any charge to her. )

The trouble began almost the moment we walked out of the doctor's office. She began to rail against him saying that he couldn't tell her what to do, he wasn't the boss of her, that she would get another doctor.

Do I even need to say that she refused to have Diane for the evenings? She insisted that she didn't need her and that Diane wouldn't have enough to do.

Flash forward to February 2009. At this time I wanted to have my mom start paying Diane. I had the idea that Diane could tell her that the agency she worked for only offered the services free for six months. Now that six months were up, my mom would have to pay. I knew that having my mom pay would be problematic even though she had become very comfortable with Diane and was relying on her to do many everyday things, like sorting through her mail (which tends to pile up and up because she doesn't know what to do with it) and helping her pay bills.

At first my mom seemed to accept that she would have to pay Diane. But within two weeks she decided she didn't need Diane every day and would reduce her time to twice a week. After a couple more weeks she got very angry about having to pay her and ordered her to stop going to her house.

Everything came back to the money. In addition to the usual accusations, she added a new one: I had spent all her money. Every day I was there, sometimes several times a day, I had to answer these charges.

Then something happened. In February she said that I had changed, and I was like the person I used to be. I asked her what she meant, and she talked about how I had taken her money and spent it, but that was all forgiven now because I've changed. I told her I hadn't spent her money, that it's safe in the bank, all the things I usually say. But it didn't seem to matter. Mom seemed convinced - like I had been a bad girl and now I was good again.

She no longer mentioned the money every time we talked on the phone. By the time I went to visit in May, it was like I was living in a different reality with my mom. She was sweet and loving to me. After I showed her a bank statement, she broke down and cried and asked me to forgive her.

Everyone told me it would get easier, but it was hard to believe when I was going through the worst of it.

The downside is that she's better with me, but she's worse in general. There are all kinds of little signs:

* More problems with language, although most people wouldn't notice because she's still fairly adept verbally.
* Less care in her appearance - she's almost completely stopped wearing makeup and tends to wear the same outfits over and over. Here too most people wouldn't notice because she's clean and well-dressed.
* A coarsening of her public behavior. After lunch one day she put not only her leftovers in a box but also mine and all the chips and salsa that were on the table. At this same lunch she told the Latino waiter a story about how she got sick one time from eating chili in Tijuana, a story that included diarrhea and chili made from cats.
* Continued problems taking her meds even though the pharmacy delivers a bubble pack once a week to help cut down on confusion.
* Odd changes in attitude toward people. For no apparent reason she's decided she no longer likes her doctor.

I'm so relieved that she's no longer angry at me. We talk on the phone every day, and she's always very sweet to me. But I'm sad that this improvement in our relationship has come about through a worsening of her condition.
